Best of all, come to the meeting, and do go back and read why you should. But if you really can't, I now have advice from Daniel Sabel, Governance Manager at CILIP on how to nominate the chair of the meeting to cast your proxy. On the form, (downloads as a Word file) fill in 'the Chair of the meeting c/o 7 Ridgmount St' as your proxy.
